1. Complete the online form in full
2. At the end of the document you will be asked to print a hardcopy
3. Have this hardcopy document signed by all the relevant people
4. Send it to Afrox via:

Important Information
- Your application will only be processed once we have received the fully signed hardcopy on one of the above mentioned address
- All pages must be signed in full
- The witness signatures are a requirement for the successful processing of the application
- Ensure that you have attached all supporting documentation e.g. ID document, copy of passport etc.
